| make this report under paragraph 7, Schedule 5, of the Coroners and Justice Act 2009 and
regulations 28 and 29 of the Coroners (Investigations) Regulations 2013.

3 INVESTIGATION and INQUEST

On 218" October 2015 | conducted an Inquest into the death of Alexander Stuart Hadley (born

14/9/1993) of Tyn y Fawnog, Dinorwic, Gwynedd and Steffan Robert Vernon (born 20/10/1981)
of which revealed matters that cause me concern.

4 CIRCUMSTANCES OF THE DEATH

On 5" June 2015 Steffan Robert Vernon and Alexander Stuart Hadley went swimming in a pool

below the waterfall at Llanberis where they drowned because of the downward current caused
by the waterfall, the coldness of the water and other matters that are not relevant to this referral.

5 CORONER’S CONCERNS

During the course of the inquest the evidence revealed matters giving rise to concern. In my
opinion there is a risk that future deaths will occur unless action is taken. In the circumstances it
is my statutory duty to report to you.

The MATTERS OF CONCERN are as follows. —

(1) The two deceased men were unaware of the dangerous currents in the poo! directly below
the waterfall and the evidence indicated that other people were also unaware of the dangers. As
the waterfall is in close proximity to the village of Llanberis and is easily accessible other people
could die in identical circumstances and | am therefore of the opinion that a sign should be
erected to warn people of the dangerous characteristics of the waterfall pool.

ACTION SHOULD BE TAKEN

In my opinion action should be taken to prevent future deaths and | believe you have the power
to take such action.
