Phuket Highway District - 313 Moo 2, Krabi Noi Sub-district, Mueang District, Kabi 81000, Thailand. 1 | CORONER Martin Fleming HM Senior Coroner for West Yorkshire Western 2 | CORONER'S LEGAL POWERS I make this report under paragraph 7, Schedule 5, of the coroners and Justice Act 2009 and regulations 28 and 20 of the Coroners (Investigations) Regulations 2013 3 | INVESTIGATION and INQUEST On 22/12/15 I opened an inquest into the death of Rebecca Leanne Shaw who, at the date of her death was aged 32 years. The inquest was resumed and concluded on 27/1/17 I found that the cause of death to be: - la. Multiple skull fractures 1b Severe head injury I arrived at a conclusion of road traffic collision 4 | CIRCUMSTANCES OF DEATH At approximately 11.15am 212/15 was the driver of a Honda motor scooter with the deceased Rebecca Leanne Shaw riding pillion. Whils made a right hand turn at the entrance of Soi Khuan Yang Charoen to cross a four lane dual carriageway known as Phra Barami Road, Patong Sub District, Kagthuy District, Phucket Province, Thailand. It was then ‘nat ctor scooter was RT3589 rT struck - an oncoming Toyota Vigo pick up truck driven by iii’ Asa result Rebecca was thrown from the motor scooter and when taken to Pa Tong hospital she was found to have died from the injuries she sustained. CORONER’S CONCERNS During the inquest I heard evidence to suggest that Pe view of the oncoming traffic to her left was obstructed by a road sign and trees, itself on a bend, before commenced her right turn from the entrance of Soi Khuan Yang Charoen. | also heard that the lay out of the road had no central reservation deep enough to full accommodate a vehicle The MATTER OF CONCERN is as follows. — e To review the safety of the road conditions and road lay out at the location and to consider the appropriateness taking steps to make the location safer for traffic. ACTION SHOULD BE TAKEN In my opinion action should be taken to prevent future deaths and I believe that the Phuket Highway District for the Muang Districts has the power to take such action.
