DT1 1X0 1 | CORONER I am Brendan Joseph Allen, Assistant Coroner, for the Coroner Area of Dorset. 2 | CORONER’S LEGAL POWERS I make this report under paragraph 7, Schedule 5, of the Coroners and Justice Act 2009 and regulations 28 and 29 of the Coroners (Investigations) Regulations 2013. 3 | INVESTIGATION and INQUEST On the 13 June 2018, an investigation was commenced into the death of Rowan Lloyd, who was born on 9" December 2003. The investigation concluded at the end of the Inquest on the 29" November 2018. The Medical Cause of Death was: Ia Multiple chest, abdominal and pelvic injuries The conclusion of the Inquest was that Rowan Lloyd died as a consequence of a road traffic collision. 4 | CIRCUMSTANCES OF THE DEATH On the afternoon of 6" June 2018, Rowan Lloyd was cycling home from school. He was cycling south on the eastern pavement of Portland Road. The road traffic was heavy, and there were both pedestrians and cyclists on the pavement at the time of the collision. As Rowan reached the junction of Portland Road, Murley Road and Langton Avenue, he: collided: with the traffic signal pole and: fell to: his right,. into: the road. At that time, a Mercedes Arcos Heavy Tractor and'semi-trailer was pulling away from the signal controlled junction. Rowan fell under the wheels of the trailer, sustaining multiple injuries. Rowan was pronounced dead at the scene. CORONER'S CONCERNS During the course of the inquest the evidence revealed matters giving rise to concern. In my opinion there is a risk that future deaths will occur unless action is taken. In the circumstances it is my statutory duty to report to you. The MATTERS OF CONCERN are as follows: 1. During the inquest evidence was heard that: i. Iheard evidence that the junction of Portland Road, Murely Road and Langton Avenue is a particularly busy junction during the rush hours and during the “school runs”, as Portland Road is the main road into Portland and this junction is close to a number of schools. However, I heard evidence that there are no crossing points for pedestrians at the junction, no advanced stop line for cyclists, no cycle lane and no barriers between the road and the pavement on the approach to the traffic signals. 2. Ihave concerns with regard to the following: i. Pedestrians have to “guess” when the lights are in their favour, depending upon the road they intend to cross at the junction: there is no pelican crossing at the junction to assist pedestrians, many of who are of school age, to cross safely at this junction. ii. There is no barrier between the pavement and the road as the junction is approached from the north. This allows pedestrians and cyclists to potentially cross between cars that are held at the junction by the lights, and would mean cars travelling in a northerly direction up Portland Road would have an obscured view of those crossing the road between stationary vehicles. iii. | There is no accommodation for cyclists. Given that many of the cyclists using this road will be of school age, particular care needs to be taken to ensure that cyclists can use the road safely and be clearly seen by drivers using Portland Road, perhaps by way of dedicated cycle lanes and/or advanced stop lines at the traffic signals. ACTION SHOULD BE TAKEN In my opinion urgent action should be taken to prevent future deaths and I believe you and/or your organisation have the power to take such action. YOUR RESPONSE You are under a duty to respond to: this. report within 56 days of the date of this report, 5‘" February 2019. I, the coroner, may extend the period. Your response must contain details of action taken or proposed to be taken, setting out the timetable for action. Otherwise you must explain why no action is proposed. The proposed hatched lining on the footways have been completed. As described in previous letter this lining is hoped to encourage people to avoid using the edge of the footway on the A354 approaches to the signals. Also, | understand that a redundant lighting column on Merley Road is hoping to be completed later this month by our contractor Southern and Scottish Electric. However, there could be a delay in this work being completed. Site investigations revealed that the lighting column is connected into the same circuit as the traffic signals. As the work may require a temporary switching off of the traffic signals additional notice is required before the works can take place owing to the potential disruption created. A pre-feasibility study of the signalled junction between Portland Road, Merley Road and Langton Avenue has been completed; a copy has been enclosed for your information. This study was completed in conjunction with a wider study that is being carried out of the A354 route between Portland and Weymouth. As you will read in the pre-feasibility study, recommendations have been made for pedestrian crossings and advanced stop lines for cyclists to be incorporated into the signalled junction. However, we are not able to commit to these works being carried out as the full extent of the A354 route study is not yet known. Funding for any improvement works on the A354 will be limited and will need to be focused on areas where the safety benefits will be greatest. The recommendations of the pre-feasibility study and safety along the whole corridor will be considered as part of the route study.
