Paramedics attended and Mrs Eveleigh was still trapped in the driver's seat of the vehicle. Death was confirmed at the scene. CORONER’S CONCERNS During the course of the inquest the evidence revealed matters giving rise to concern. In my opinion there is a risk that future deaths will occur unless action is taken. In the circumstances it is my statutory duty to report to you. The MATTERS OF CONCERN are as follows. — [BRIEF SUMMARY OF MATTERS OF CONCERN] The location of this collision was the junction of Station Road with the A358 at Bishops Lydeard. | heard evidence at the inquest that a roundabout had been proposed at this junction for a number of years and that by the time the inquest commenced works were about to start. My concern is that there was a sign erected at this junction indicating that there had been a number of accidents at the junction previously but | was concerned as to whether the erection of such a sign would also necessarily involve a review of maximum speed for vehicles approaching such a junction.
